1. Fill out the Bariatric Patient Questionnaire.
   
   
2. Get documentation of all weight loss programs that you have participated in. Your insurance company will require a minimum of at least one physician supervised weight loss program that lasted at least six months or longer. Many companies require participation history in two programs, with one programs within the last two years.
   
   Documentation means the following:
   Copies of physician notes showing visits for weight loss with recorded monthly weights. If you have been part of a professionally supervised program we will need a copy of your attendance and weigh in data. Any summary letters that can be supplied are appropriate, but a summary letter alone will usually not suffice for the documentation that you have actually participated in a program.
   
   Documentation does not mean the following:
   Our goal is to get approval as quickly as possible for you. As such, a copy of all of your medical records, while informative, does not usually speed the process because there is too much information not targeted for our purposes.
   
   
3. Bring copies of any lab work, X-ray reports, or operative summaries done within the past six months. Of particular importance would be CBC and Thyroid function studies.

When you have all of the above information please bring it to the office and we will review it. You should be contacted by the office within 7 to 10 business days to schedule your appointment.
