you and your child to ask questions. Let the staff know of
any special needs your child may have. We want your child's

IMPORTANT PRE-PROCEDURE INSTRUCTIONS FOR CHILDREN
HAVING SURGERY UNDER GENERAL ANESTHESIA

with a safe, comfortable surgical and anesthesia experience.
Please follow all instructions completely.

Braid or tie back long hair.
- If your child has any change in
his or her health before the date of the procedure, notify
your child's surgeon. Report any signs or symptoms such

- Please leave all valuables at
home.
- If the patient is under 18 years
of age, a parent or legal guardian must accompany him/her.
